While most upper-class men in ancient Rome did participate in homosexual activity it was almost always as the dominant, penetrative participant which was not viewed as diminishing the masculinity of the penetrating participant. I don't know of any Roman emperors who were "transsexual". It's not a good idea to apply modern definitions of sex and sexuality to people of the past.
